Cost of Services (Pricing), & Payments
Initial Comprehensive Visit to Establish Care:
Whether you are establishing care as a Primary Care Client, Medical Weight Loss Client, or Bioidentical Hormone Replacement Client, it's important to Dr. Alexis Hanson, DNP, FNP-C to really take the time to get to know you. For your first visit, Dr. Alexis spends one to two hours to review and discuss your chief concerns, medical history, and goals. Typically, she likes to order lab work and research your case after your initial visit. This will allow her to give you the most comprehensive care. This Initial Comprehensive Visit is typically an In-Office Appointment. Your follow-up appointment after the initial visit, can be scheduled as an In-Office visit or a Virtual (Telehealth) visit, and is scheduled one to two weeks after the initial appointment, then additional follow up visits will be scheduled after that, dependent upon your individual health issues. The cost of the initial visit depends upon the time and complexity of the visit. Most new patient visits are 1.5 hours.
Dr. Alexis Hanson, DNP, FNP-C: $265 for 1.5 hours.
$89.00 for additional 30 minutes for new patient visit.
Follow Up Visits:
Follow up visits with Dr. Alexis can be done as Virtual (Telehealth) visit, In-Office appointment, or via phone. These are a great resource to established in-town, and out-of-town clients. Follow up visits typically costs between $89-150. The fee is based upon the time and complexity of the follow-up.

Non-Established Client (or New Client) Visit: If you live locally in Arizona, or just visiting from out of town and need to see a healthcare provider for care or treatment, Dr. Alexis and her team can help! Appointments can be done by Virtual (Telehealth) or In-Office.

$189 for 30-60 minutes
Pre-Op (Preoperative Medical Evaluation): If you are planning a surgery that requires Pre-Op Medical Evaluation (including Physical Exam, Labs, and Diagnostic testing), Dr. Alexis Integrative Health can help. This Pre-Op Medical Evaluation is a two-part visits, and can be scheduled with Dr. Alexis as an In-Office visit or as a combination Virtual (Telehealth) visit and In-Office appointment visit.
Established Primary Care Client: $275
Non-Established Client: $375
Email/Referral Correspondence: We strive to make our care as convenient as possible for our clients. However, any email, referral, report, document request, or consult that requires a more complex answer or takes more than five minutes of the doctor’s time may carry a fee similar to a visit.
We offer 15 minute Virtual or In-Person Complimentary Consultations for all Services (except Primary Care).

IV (Intravenous) & Injection Therapy:
IV Nutrient Therapy: Typically ranges from $175-$250 dependent upon types & amount of nutrients combined.
B12 Injections: $20 per injection (shot), or Buy 4 & Get 1 FREE.
Immunity Injections: $25 per injection (shot), or Buy 4 & Get 1 FREE.
Lipo-Mino (FatBurner) Injections: $25 per injection (shot), or Buy 4 & Get 1 FREE.
PAYMENT FOR SERVICES:
Payments for services at Dr. Alexis Integrative Health are due at the time services are rendered. Accepted forms of payment are: Cash, Visa, Mastercard, Debit Cards, HSA or FSA, American Express, and Discover Card.
We do not accept payment from insurance companies or bill insurance companies for the provider visit. However, we do our best to comply with insurance requirements so that you can get reimbursement. Let us remind you that submission of a claim for reimbursement does not guarantee reimbursement and is subject to health plan benefits. To help you in your quest for reimbursement we will do the following:
Provide you with a “superbill” that summarizes the services rendered, diagnoses, and charges applied. The “superbill” is a document (similar to an insurance claim form) which you can then submit directly to your insurance company for reimbursement. Most integrative health providers are considered to be out-of-network providers, so you will want to check with your particular insurance company on their out-of-network policy.
Gladly fulfill any requests for additional forms from your insurance company.
Give the copied information to you to submit to your insurance company (along with their letter of request). We HIGHLY recommend that you only send copies of the documents, never the originals.
Please note that if we receive letters from insurance companies requesting records or different diagnosis codes, different CPT codes and additional information we will give this additional information to you to submit to your insurance company; however, we will not send the information directly to the insurance company. Any payment for insurance reimbursement should be sent directly to you, the client. Call your insurance company to request claim forms to submit for reimbursement.
Labs & Diagnostic Testing:
Labs and diagnostic testing are generally covered by your insurance company when their in-n etwork labs or diagnostic centers are used. Check with your particular plan. We can recommend a low-cost, cash-pay lab or diagnostic center, when labs or diagnostic testing are required but are not covered by insurance or if you do not carry insurance.
If your provider at Dr. Alexis Integrative Health orders lab or diagnostic testing, we provide you with an order, allowing you to schedule directly with the lab or diagnostic testing center of your choice, such as Sonora Quest, LabCorp, Radiology Ltd., and others. You can then schedule your visit, and give them your insurance information up front, so they can bill your insurance directly for these tests.
Flexible spending accounts (FSA) / Health savings accounts (HSA):
Many personal FSA or HSA accounts will cover some of the services offered at Dr. Alexis Integrative Health. Please check with your specific plan.
When you book an appointment or consultation at Dr. Alexis Integrative Health, we exclusively reserve that time for you with the doctor, nurse or staff, and facilities. We ask our clients to give us at least a 24-hour notice if they need to cancel or reschedule their appointment, which allows us time to fill that open appointment slot with another client. If you no-show for your appointment or cancel/reschedule your appointment without the sufficient 24-hour notice, a $75.00 fee will be charged to your account, and you may be required to pre-pay for your next appointment.
